Your primary care provider can simply make a referral to our department and our referral coordinator will contact you to schedule as soon as possible.
You will receive a new patient appointment letter in the mail which will contain, directions to the office, insurance coverage steps, and additional paperwork that will be helpful to have available at your first appointment. Please complete all forms that will be included in the packet.
Note that some of the questions may not be relevant to your child's situation; if so, leave those parts blank. Thank you for taking the time to gather these materials if necessary, and we look forward to seeing you at your appointment!
Medical Records: If your child is being evaluated for developmental, attention, or learning concerns, please bring all evaluations, IQ tests, and any academic achievement testing that was completed by a teacher or professional with you to your child's appointment. This will allow us to better understand your child needs and develop treatment plan, medication management for developmental (including Autism), learning (including ADHD), and any behavioral concerns.
